The Chicagoland Chamber of Commerce (Originator) is responsible for the contents of this survey. Your response to the survey is voluntary. If you are asked to reveal your identity or the identity of the organization on whose behalf you are responding and do not wish to do so, please do not respond to the survey. Any personally identifiable information, such as your e-mail address, name, home or work address or telephone number, that you provide in connection with responding to a survey may be used by the Originator (or a third party acting on behalf of the Originator) to follow up with you and otherwise in accordance with the Chicagoland Chamber of Commerce’s Privacy Statement at www.chicagolandchamber.org/privacy-statement.
If you are given an opportunity to submit free-form responses to this survey or any survey from the Originator, please provide relevant and responsible responses; refrain from disparaging, offensive, harassing or otherwise abusive or inappropriate language. In responding to any survey, please ensure that you do not upload or otherwise transmit any material that contains software viruses or any other computer code, files, or programs designed to interrupt, destroy or limit the functionality of any computer software or hardware or telecommunications equipment.
Do not forward any survey without the permission of the Originator, and please contribute to its effectiveness by responding only once.
The Chicagoland Chamber of Commerce does not want you to, and you should not, include in your survey response any sensitive information of any kind (e.g., credit card or bank account information), and any confidential or proprietary information belonging to you, the Chicagoland Chamber of Commerce or any other party, as the Chicagoland Chamber of Commerce cannot guarantee that access to such data will be kept confidential and secure.
